Your legacy care journey, step by step
1. Complete your pre-qualification quiz
Taking our quiz determines your eligibility for your selected medication.
A successful quiz result pre-qualifies you for your selected medication, which allows you to make a purchase.
All your information shared at this stage is kept private and confidential. We are a fully HIPAA-compliant company, and any data you share is encrypted and not accessible to anyone outside your care team.
2. Choose your plan and submit your medical questionnaire
Upon purchasing your medication, you will be asked to fill out a more detailed medical questionnaire which requests relevant health information as required by our licensed clinician assigned to you.
This medical questionnaire is crucial in order for us to be able to ship your medication. You will find a link to it in your email inbox; just search for an email with the subject line: One More Step Before We Ship Your GLP-1 Medication
Your medical questionnaire can also be accessed from your Medical Portal.
To access your Medical Portal, please make sure you use the email address you used for the purchase. No password setup is needed, just login with the one-time passcode (OTP) emailed to you.
3. Your licensed clinician reviews your information
After submitting your medical questionnaire, your assigned licensed clinician will review your information and determine whether a prescription is appropriate for you. If found unqualified, your order will be cancelled and refunded.
4. The pharmacy prepares your prescription
Once your assigned licensed clinician issues a prescription, it is automatically sent to one of our partner pharmacies to prepare your medication.
Orders are typically shipped within 3 – 7 business days.
5. Track your order
Once your medication is shipped by our partner pharmacy, you will receive the tracking information via email. The tracking details will also be made available in your Customer Portal.
6. Start treatment as prescribed
Your order contains everything you need to begin. Follow the start date and instructions provided by your licensed clinician.
Make sure to store your medication as advised, and follow your treatment plan as prescribed by your licensed clinician.
For guidance, review the digital guide sent to your email. It includes details on how to take your medication, how to store it, potential side effects, and more.
7. Reach Support for account or order questions
You’re never alone on your journey, our dedicated care team is available to support and answer all your questions, Email help@joinsprouthealth.com or call +1 (833) 496-4020.
8. Ask your clinician about your care
If you have any questions about your treatment, side effects, or dosage changes, we recommend contacting your licensed clinician through your Medical Portal here.
Important: If you have previously used this medication and wish to continue at a specific dosage, please inform your doctor during your consultation before your prescription is issued.
9. Request your medication refill
Based on the program plan you purchased (monthly or quarterly), you will be charged next month or in 3 months.
Monthly rebills & medication refills
- Monthly subscription plans are rebilled on the same date each month. This is a recurring subscription: the charge amount, billing frequency, and billing date are shown and confirmed at checkout before your first payment. You can cancel anytime before your next rebill.
- After each rebill, you will receive a new medical questionnaire to fill it out. This step is essential for your licensed clinician to evaluate your progress and assess any possible side effects. Your licensed clinician shall determine any increase of dosage as they deem appropriate. We can only ship your refill once the new refill questionnaire has been submitted, and a new prescription is approved by your licensed clinician.
- If you experience any side effects, have questions about your medication, or need medical guidance, message your provider through your Medical Portal.
Quarterly rebill & medication refills
- Quarterly subscription plans are rebilled on the same date every 3 months.
- The refill for quarterly plans is different for each personalized treatment offered by Sprout Health:
- For GLP-1: You will receive medication for a 2-month period only; and will be eligible to submit a new medical questionnaire after the third dose of your second month.
-
For NAD+: You will receive medication month-by-month, so please remember to submit your medical questionnaire after the third dose of every month.
Compounded GLP-1, GIP/GLP-1, and NAD+ medications are not approved by the FDA and have not been evaluated for safety, effectiveness, or quality by the FDA. - Submitting your medical questionnaire is essential for your licensed clinician to evaluate your progress and assess side effects. Your licensed clinician shall determine any increase of dosage as they deem appropriate. We can only ship your refill once the new refill questionnaire has been submitted, and a new prescription is approved by your licensed clinician.
- Your refill questionnaire will be available for you to fill out in your Medical Portal.
- If you experience any side effects, have questions about your medication, or need medical guidance, simply message your licensed clinician through your Medical Portal.
You’ll be eligible to request your next month’s prescription after your third dose in the current cycle. This allows our medical team to monitor your response and dosage needs effectively.
We will also send you the refill form via email.
